Waiver, Consent, and Releases

I hereby grant permission for the named on this form to participate in the activities of the Forest Hill T-Ball Association and agree to insure my child against any loss, injury or damages resulting from these activities. I agree to indemnify and save harmless the Forest Hill T-Ball association, its executive members, umpires and volunteers from all losses, claims, demands and lawsuits occasioned thereby. Further, I hereby authorize the coach to secure medical advice and services as deemed necessary for the health and safety of my child. I agree to accept financial responsibilities in excess of the benefits allowed by Provincial insurance.
